  • Rarely (1 point)
  • Sometimes (2 points)
  • Often (3 points)
  • Very often (4 points)

Result Interpretation

  • ✅ 0–13 points: Low Risk – Your current risk of cardiovascular disease is low.
  • ⚠️ 14–26 points: Moderate Risk – Some risk factors are present; improvement is recommended.
  • 🔥 27 points or more: High Risk – Immediate lifestyle changes or health management may be necessary.
